unable to see line manager role in worksheet process.

We have configured Primary hierarchy formula four level which has been working fine. However, for secondary level Hierarchy defined only one manager but system not showing in the process. all three person have approved the worksheet but when i logged in through secondary manager then sheet still showing in updateable mode.

Is this not wrong when all level primary level approved but sheet still appearing updated mode at Secondary line manager screen.

Secondly, what is the role and how primary manager hierarchy people know that secondary manager first perform action and then primary manager will either override secondary manager consideration or allow it.
