Request Feedback set the Provide Feedback By Due Date

Need to have a specific feedback date for solicited feedback.

We would like to set the Provide Feedback due date much in the same way as we do with Participant Feedback in the Performance review. Has anyone done this with a page personalization ? Can you share ?
