What is the right way to create an OIC application on OFS ?

Summary:

I am trying to create a OICS application to send activity update from OFSC to B2B Service. As per the documentation I need to fill Application type (Oracle Integration), Application name, Host, Username and password. But while creating the application on OFS I can see that there are various required information which are unknown to us.
