Not able to view the Check-In document details in Annual Performance document

Content (required):

We are going to implement the Check-in documents.

I have created the check-in documents and assigned to the employee.

And also enabled "Display Check-ins" in Performance template and enabled "Include in Performance document"

in check-in template configuration.

When i click on the check-in document in Overall summary section in annual performance document, it is redirecting to PDF format and details are not showing up properly.

Attached the PDF file for your reference.

Could you please help me on this issue.
