Custom Performance document Notifications are not viewed

Hi,

we created a custom version of the performance document notification following the pieces of information shown in the Doc ID 2647434.1 (the performance document notification I referred to, is the following one: 'PerformanceApprovalReport'). However when I open the approval notification (via worklist) of our performance document, sent to the approver, I do not see the custom version of the notification, but I see a blank page (see the screenshot I attached).

Hoping someone has some suggestions.
