Updating performance history with questionnaire results

The performance document is consisting only a questionnaire ( without goals and competencies sections) for both employee and manager. Manager provide a final performance rating level for the employee when completing the questionnaire. When preparing the questionnaire I have given performance rating model for the question which asking about the employee performance level. After completing the evaluation employee performance details and history ( graph in the performance tab) in the spotlight are not updating with the performance rating . Is there a way show the employee performance history when using a questionnaire ?
