Implement Performance Improvement Plan

For the performance improvement plan, I have the following process requirement:

Manager states the issue and the desired outcome (thinking this can be a questionnaire) in the application

The employee will then meet with the manager. They will see the manager inputs of the issue / desired outcome and enter their action plan in the application

After the observation period, manager can go in and give the final rating of the PIP results (pass, fail, etc.)
Functionality wise, we can’t have employees view the manager comments to questionnaire as a first step before they can take the action and enter their action plan (PIP goals). I am interested to hear from others how you implement PIP in Oracle HCM.
