We have a requirement to create a Flex Field of Username in Create Service Request Page. But, I'm not knowing which Module or Name I should choose to create on the Manage Descriptive Flex Field page.

I have attached the reference screenshots on the requirement, please check and let me know any possibilities that are exist for this requirement.
