Quick Links to performance document Questionnaires

Summary:

Our requirement is to provide the ability for managers to go directly to performance Questionnaires for a month from the performance landing page of the employee rather than going into the monthly performance document and then selecting the Questionnaire for the month saving it.

So we are trying to skip one step in the process so that managers can reach the monthly questionnaire through a quick link. Is this requirement posible?
