How to check when the performance document was assigned to employees

Hi Everyone!

Is there any way to check when(date) the performance document was assigned to employee/employees.

Context:

Initially we assigned the performance document to employees in January and in April we cancelled, deleted and again assigned same performance document with adding questionnaire section to employees.

Need to fetch the date when performance document was last assigned.
