How to add Free Text Field on Request feedback Page

Hello ,
Kindly advise do we have the option to add a free text box in which the employee who is requesting feedback can add a few words for what he is requesting feedback?
This text (Along with the Questionnaire) should then be visible to employees from whom feedback is requested.
For example, if I were a user and received a general request for feedback, I would want to know what it is I am providing feedback on, i.e., a specific project name, presentation, etc.
