In performance document, questionnaire section refreshes automatically

Summary:

In performance document, questionnaire section refreshes automatically. Some employees have had multiple page refresh by itself. Probably around 10 over 30 min. Each time, they lost some of the content they wrote.

Is there a way to resolve this issue?
