Enrollment Error - Rate amount you entered is 0 but won't open to let me enter rate

I have never seen this before. I am doing a Ben Admin correction. I have a life event called Benefit Administrator, with the following enrollment rules:

However, when I click on the new option I get:

And it won't open to let me put in an amount. This is for an HSA plan. And the other issue is where is it coming up with 600-6384??
