Is there a way to view the check-in document notes from the year-end evaluation?

We have configured a check-in template (new 18B functionality) and completed a check-in as an employee. For example, the employee completed the questionnaire and added a performance goal with notes as a discussion topic.

Is there any way to view the check-in notes/details from inside the employee's year-end performance document? We are looking for the ability to view the questionnaire and discussion topic responses from the year-end evaluation, so that it can contribute to the year-end review content.
