Remove fields on RMI Questionnaire

We use an RMI questionnaire to capture SSN and DOB information. We do not need to capture the Expiration Date of the SSN. When it is populated, it causes an adverse downstream impact to other systems.

1. Is it possible to remove this field from the RMI Form?

2. Is itpossible to add text (e.g. help text) to the Form to inform the candidate not to populate that field?
