Option to calculate rate based on number of child and the age of child

Our client has coverage rates based on Age of the Dependents which includes both Child and Spouse.
An employee can enroll up to three children in a plan. Based on the age of the children the rates will differ.

An employee can enroll up to three children in a plan. Based on the age of the children the rates will differ.
For example:
Child1 age is 4 and the rate is 100 (If Child's age is between 0 to 5 then the rate = 100)
Child2 age is 7 and the rate is 150 (If Child's age is between 6 to 10 then the rate = 150)
