Warning Message for selected Performance Rating in the Performance Document

Summary:

Hi. We have a client requirement for a Warning Message to prompt the Manager to add individual comments to the goals included in the performance document when he selects either a 1 or a 5 as the rating for this goal. Is there any way we can achieve this?
