Is it possible to add action reason field (e.g. termination reason) in potential duplicate screen?

Summary:

We have a requirement to include the action reason (specifically termination reason) in the potential duplicate screen. When checking for duplicates, and the potential duplicate screen appears, business wants to see if the candidate was a voluntary severance leaver so that they won't be offered anymore.
