Candidates documents not visible in DOR even after them uploading it in journeys

The ideal behavior is candidate goes to their journeys and upload the documents required and the documents get uploaded to document of records automatically.

However recently we have been facing certain issue where candidate would have uploaded the documents and completed the task and the document is visible in DOR, but once he goes to the next task the previous task is still showing incomplete( even when he has completed the task which is why the documents are reflecting in DOR )
