I created questionnaire with 2 questions, Feedback is not taking for the answers if it was "NO"

Hi People

I created a questionnaire with 2 Questions and added to Checklist, both the questions has only two options whether yes or no, if the response is no for the question 1 it should ask for feedback why No, even the same for Question 2.. But the issue for us is it was asking feedback if both the 2 questions responces were "No" .

It worked in Dev1 and Dev4 but not working in Test Instance!

Can someone please help me in this
