Unable to add questionnaire in Performance section , section type :- questionnaire

Hi

We have created questionnaire and need to enable it for employees in pms document through Performance section however the created questionaries are unable to be fetched in performance section , section type created as questionnaire

Is there are other way to do that, appreciate quick response
