Offboarding Checklist triggered without action

Hi,

I have an offboarding journey checklist for when employees resign or are terminated, however I noticed a few employees are getting the checklist assigned to them without a termination or resignation action on them. These are active employees. Has anyone seen this behavior? Any steps on how to remedy this or find out who triggered the checklist?
