The outcome, resolution and description fields are no longer exposed in the Edit HD request page for Agents. How can we expose this group again using the dynamic form in Visual Builder?

The agent is no longer able to view the Resolve Edit template in the Edit HRHD request and not able to select the values from the Outcome drop-down, Resolution Code drop-down and add the resolution description.
