Checklist template with task type Electronic Signature error (PER-1532712)

Hello everyone, 

I am having this issue related to Journey. I have this checklist template with task type Electronic Signature and I have specified the Report Path. 

I managed to allocate the checklist to the selected employee. But, when I click on the document, an error appears below: 

"The report doesn’t exist in the specified report path. Contact your HR representative. (PER-1532712)"

I have searched various sources, but have not found the answer I am looking for.

Does anyone have any ideas what might be causing this to happen? 

Any advice appreciated. Thank you.
