Digital or Electronic signature not displaying on printed performance form

Summary:

When printing a performance eval form, the signatures and dates do not appear. We need to have this appear for JTC survey reviews. how can we get these to appear on the form? We have display digital signature box checked.

It will have the employee name in the phrase 'Electronic signature confirmation provided by ___' but we need to have both employee and manager signatures displayed with dates.
