Document of Record - Enter details before E-Signing the Doc

Hi All,
We have a requirement to configure a Document of Record with E-Signature where user need to provide few inputs before signing the document.
1) We created a Journey for this and one of the task in Journey is configured as E-Signature (Native method) .
2) . Custom BIP report is configured for this task. It is working as expected and users are able to sign document electronically and document is stored as DOR after completion of task.
3) There is a requirement that Employee who is signing the document, should provide some information (for e.g. see fields highlighted in red) on the form before signing it. Since the report is opening in PDF format which is non editable & user is not able to provide required info in it.
