Performance Rating Real Time Subject area gives duplicate data for a new Performance form

There seems to be an issue while creating a report and fetching data using Performance Rating Real time subject area. The feedback completion date is coming as blank in one row and with a value in second row. I just wish to have that row fetched where there is data.

However, There are some cases in which there are blank values for this field so we cannot exclude blank values by default.

Can anyone please help on how we can fetch correct data removing duplicate values.
