How to show the rating model description used in Questionnaire inside the performance template?

We have a new template of performance appraisal that needs to have both behavioral indicators and Rating descriptions. I know if I used competencies the rating description of the proficiency will show under the rating. However, the behavior indicators are not. That's why we decided to use a questionnaire. However, in the questionnaire the rating description is not showing.
