Customer name gets altered while updating bank account details of a site

Hi,

We are trying to figure out the reason for customer name changes that are being happened in the production environment frequently.

We have been observing that the organization name is being updated with the collector's name and the last updated records show the collector details.

The collector confirms that she has been only updating the bank account details on a site and not sure how the organization name gets changed.
