Initiate Journey automatically when Performance evaluation is less than 1.49

Summary:

Expectation:
• When an employee receives an overall score of 1.49 or less on their Annual Performance Review, it should trigger a Journey as the form of notification.

Actual:
• The journey did not get triggered even though the completed annual review has a 1.36 overall rated.

Does anyone have steps to configure this so that the Journey gets automatically triggered when the performance evaluation is complete?

Note: currently the Performance is coming separately and not from the Person Profile.
