Staff Account Access Report

I am currently running the out of the box report:  Staff Accounts by Profile with Last Accessed - this report is not currently functioning for us- it processes for close to 9 minutes then errors out.  I am in the process of submitting an SR.

What we are hoping to produce is a report that will show us by month and by staff account unique log ins.

Example

  Jan Feb Mar Apr May Jun Total
Michael 21 44 20 22 0 0 107

Does this kind of report exist? I'm trying to build it, but am having trouble starting.
