Adding Employee Questions Section to Performance Template

Hello everyone,

I’m working on customizing a performance template for my client, and they want to include a section where employees can provide feedback on their goals for the year, like "What is the most valuable contribution I can make to the organization?"

I’m considering creating a Performance Template Section with a questionnaire type to include these questions. Do you think this is the best approach, or is there a better solution?

Additionally, I’m having trouble locating where to select or add the questionnaire within the performance template. I can’t find an option to choose it either in the template selection or within the performance template itself.
