REDWOOD: Not able to request feedback as HR Specialist

Hi All,
Getting the errors in the screenshot below and " An unexpected error occurred" while attempting to request feedback for an employee navigating through My Client Groups → Quick Actions → Feedback/ Request Feedback. However, the employee still could able to access request feedback from their end.
