Audit of agent/user transactions

Hi colleagues,
There appear to be out-of-the-box reports that can tell what an agent has done within a customer record. However, I can't find a report that would tell me when someone accessed a customer record but didn't leave a note or interaction outlining the reason for accessing the record.
Is the above available from an out-the-box report or is there a way of identifying such records.
