Fillable Document of Records as one of the certifications for a Plan?

Our client has a transport voucher election which has few fillable details like "Name", "Subway card or Bus pass" etc. Currently they are receiving it as a document and then manually enter.
The certificates we upload go directly to the DOR as attachments. Is it possible to add a DOR and then it fulfils as a mandatory certification?
Employee can create a DOR and fill the details from the Document Records section and the Admin can approve/reject it accordingly
