answer feedback report with answer-ID, Question text, incident number and incident text

I want to create a report showing all individual feedback per answer (preferably even a report that enables me to filter on either answer ID or product or both). However, I have no clue how I can link the answers to the incidents with the feedback.

The report would have to look like this:

Rollup columns:

- Answer ID

- Summary


Incident number

Incident thread

I might want to add a date filter also, but for now I would be very happy with a report that links the answer to the individual feedback.

I hope someone has managed to create such a report and can help me out...
