All Survey Responses in One Row, Without Crosstab

We are trying to export our (transactional) survey results as a CSV file, and then have the results loaded into a different system for further analysis. The other system cannot ingest the report with crosstab formatting. If I remove the crosstab format, however, the report breaks a single survey into multiple rows - one row for each question response on the survey. Does anyone know how to get one survey's responses into a single row? I've attached a report definition, as well as a screenshot of how the results return in multi-rows. Thanks for any help you can provide!
