Performance Review content disappear after saving and submitting?

Summary:

We have launched our performance reviews in 24C. Our employees have entered their performance data, selected save and submit. The reviews are in the manager review task. When the manager goes to view the content all of it is blank, when I go to review as an admin it is all blank. If we pull an archive report the content the employee entered in the system is there, however the review is still blank in the system. We have now had managers try to input their feedback and those are starting to disappear as well. This is a widespread issue
