Approval on Performance Rating

We have a requirement on approval regarding performance ratings: There are two rating levels, On Track  and  Off Track. If a manager gives an  Off Track rating, it should trigger an approval request to HRBP; otherwise, no approval is needed. Is this possible to achieve
