Checklist task being assigned to wrong user

Hello,
User A is an employee from location Kurla and has no AOR mapped in her profile , but when a checklist task is tagged to an pending worker User B from reality location the journeys tasks notification is being triggered to to user A who isnt even having any AOR or is not even related to the employee. Three is the reminders which is being sent to user A regularly and clogging the emails of the user A.
How can we check that ?
