Historical Performance Management data

Our organization has currently uploaded their historical performance data in a custom profile content section which is a part of 'Profile Management' module but we wish to migrate the same to the 'Performance Management' module only so that employees, managers & PMS admins can easily navigate to view historical and present performance ratings of the employee.

Please share any ideas/approaches to achieve the above shared requirement.
