Performance Customization - How to change label for the Questionnaire on talent profile?

We would like to change the label of the Questionnaire on the Talent profile of the employee

When a user gets evaluated on a Performance Document, and his Manager accesses to his Talent Profile, he can see this:

and then:

We would like to change both labels on "Questionnaire" title, and "My Questionnaire" section title, but we want to perform this Customization specifically for one document type and not for everyone.

How can we do this?
