FF for EOI Requirement for Short Term Disability Plan and different Options

If Employee is previously not enrolled in Short Term Disability Plan and currently getting enrolled in any of the below options, employee should be providing the EOI:

90 Days

60 Days

30 Days

15 Days

If employee is currently enrolled in STD Plan and any option, and changing the option to higher ones,

employee should be providing the EOI
