Exception report when incident updated X times

Hi everyone

I would like to create an exception notification (ideally sent by email) when any incident is updated X times (probably 2) by the customer. I would like this to happen in real-time as much as possible.

This is to allow the manager to immediately look at the incident to see why it has not been resolved already.

How would I best achieve this?
