Adding AHT to report

Hi 

I have hit a bit of a stumbling block with a custom report.

I have attached a Word document showing a few screen prints and some of the definitions I'm using. Would someone mind giving this the once-over for me and letting me know where I'm going wrong?

It would be fantastic if I could get this working.
