Get multiple answers in one row for feedback

Hi,

We have a survey with 3 questions something like:

1. Why did you leave this feedback? (Several checkboxes)
2. What rating do you give? (Several radio buttons)
3. Comments (Textarea)

What I am after is a report or some way to list all comments, the date and the score the customer gave.

In ROQL I can see doing this with two loop statements but this wouldn't be my preferred way.

Any suggestions?
