Performance: Questionnaire not showing in list to be added in check-in template

Hello,

Under questionnaire, I used subscriber 'Performance' to create a check-in questionnaire.

Now, under the check-in template, i cannot see the questionnaire to add it for worker or manager.

Please advice.
