Month wise incident report

HI,

I would like to create the schedule report for month wise for last one year. The columns are no.of created incident,no. of pending incident and no. of closed incident. Kindly let us know how we can build the report in below format. 

Month Created Pending Closed
Jan-2016  10 5 3
Feb-2016 14
 		 10 8
