Report to show custom field usage

I'm trying to put a report together that shows me how many times a custom field is used. The rationale being that I want to remove any unused fields!

I fail to see a table that can accomplish this? Ideally I'd want to see not only how many times a field is used, but also where the field is used (eg which workspace).

Can anyone point me in the right direction?
