Missing Goals After Approval

Summary:

Recently a team of 14 members submitted their goals for approval, but half of them faced issues:

Scenario 1: An employee submitted three goals, all of which appeared in the approval notification. However, after approval, only two goals are visible on the Goals page.

Scenario 2: An employee submitted three goals, withdrew the approval to make further edits, and resubmitted. After approval, no goals appear on the Goals page.

I would like to know if anyone else has experienced similar issues. I plan to proceed with opening an (SR) to address them.

Our company goals, totaling approximately 9,000 EEs, were due in the system on April 1st. We did not encounter any cases related to this issue until our recent transition to 25A.
